曲安奈德注射联合激光治疗对皮肤血管瘤患儿血清VEGF和HIF-1α的影响

         

摘要

目的:探讨曲安奈德注射联合激光治疗对婴幼儿皮肤血管瘤患者血浆血管内皮细胞生长因子VEGF及缺氧诱导因子HIF-1α表达水平的影响.方法:收集如皋市人民医院皮肤科门诊接受曲安奈德注射联合激光治疗的增殖期婴幼儿腹部表浅血管瘤30例患者为治疗组;对照组:30例,为笔者医院儿科正常体检婴幼儿.各组婴幼儿的外周血血浆中VEGF及HIF-1α浓度利用酶联免疫吸附分析法检测并进行浓度变化动态观察.组间比较采用两因素方差分析,两两比较采用LSD法.结果:对照组婴幼儿血清VEGF浓度水平为(257.391±76.612)μg/L,HIF-1α为(2.975±1.045)mg/L;治疗组婴幼儿患者治疗前血清中VEGF浓度水平为(416.849±104.219)μg/L,联合治疗4周后浓度水平为(285.402±122.457)μg/L(P<0.05),治疗12周浓度水平为(241.873±115.694)μg/L(P<0.05),血清浓度水平下降明显;婴幼儿患者治疗前血清中HIF-1α浓度水平为(12.347±1.463)mg/L,联合治疗4周后浓度水平为(7.425±0.877)mg/L(P<0.05),治疗12周后浓度水平为(2.414±1.948)mg/L(P<0.05),血清浓度水平也呈下降趋势.结论:婴幼儿皮肤血管瘤患者接受曲安奈德注射联合激光治疗后,血浆中VEGF及HIF-1α浓度显著下降;VEGF及HIF-1α有望成为评判曲安奈德注射联合激光治疗婴幼儿皮肤血管瘤效果的客观检测指标.%Objective To evaluate vascular endothelial growth factor (VEGF) expression and hypoxia-inducible factor 1α (HIF-1α)expression in patients'serum treated with laser combined with injection of triamcinolone acetonide. Methods 95 proliferating hemangioma infantstreated with laser combined withtriamcinolone acetonide from January 2014 to October 2016 were engaged in this study. 30 normal control infants collected from the department of pediatrics served as the healthy controls. VEGF and HIF-1αexpression in the plasma were measured by ELISA. Statistical analysis was done by two-way analysis of variance followed by the least significantly difference (LSD) test. Results The serum level of VEGF in control group was (257.391±76.612 )μg/L, and the serum level of HIF-1α in control group was (2.975±1.045)mg/L. The serum levels of VEGF and HIF-1α in hemangioma patients before treatment were (416.849±104.219)μg/L and (12.347±1.463 )mg/L respectively.After 4 weeks treatment, the serum levels of VEGF and HIF-1αwere decreased to(285.402±122.457)μg/L (P<0.05) and (7.425±0.877)mg/L (P<0.05).After 12 weeks treatment,the serum levels of VEGF and HIF-1αwere decreased to(241.873±115.694)μg/L (P<0.05) and (2.414±1.948) mg/L (P<0.05) respectively. Conclusion The serum levels of VEGF and HIF-1α were decreased significantly after the laser combined with injection of triamcinolone acetonide treatment. VEGF and HIF-1α may be the objective index to evaluate the effect of the laser combined with injection of triamcinolone acetonide treatment on infant hemangiomas.
