CT对累及门腔间隙淋巴结淋巴瘤和结核的r鉴别诊断价值

摘要

目的:探讨CT对累及门腔间隙淋巴结(PCNs)淋巴瘤和结核的鉴别诊断价值.方法:收集经病理证实的22例累及PCNs的腹腔结核和32例累及PCNs的非霍奇金淋巴瘤(NHL)的临床和CT资料.观察增大PCNs的解剖分布、形态、强化特点等.结果:①PCNs受累主要表现为门腔间隙下份单独受累或其上、下份均受累,2组差异无统计学意义(P>0.05);②PCNs呈均匀强化者多见于NHL,而PCNs呈环状强化者多见于腹腔结核(P<0.01).结论:CT可良好显示结核和NHL累及PCNs的细节,可为两者的鉴别诊断提供帮助.%Objective:To evaluate CT value of differentiating the portacaval lymphadenopathy in patients with tuberculosis and lymphomas. Methods:The CT images of 22 patients with tuberculosis and 32 patients with untreated non-Hodgkin's lymphomas (NHL) were reviewed retrospectively. All cases showed the portacaval nodes (PCNs) involvement. Great attention was paid to the anatomic location and contrast-enhanced findings of the PCNs. Results:The involved PCNs located in the lower part or all the part of the portacaval space (PCS). The anatomic location of the involved PCNs had no difference between the tuberculosis and lymphomas (P>0.05). The enlarged PCNs with homogeneous enhancement were more often detected in the patients with NHL (P<0.01). The enlarged PCNs with peripheral enhancement were more often detected in the patients with tuberculosis (P<0.01). Conclusions:These findings indicate that the anatomic location and contrast-enhanced appearances of the PCNs on CT images can be useful in differentiating between tuberculosis and NHL involving the portacaval nodes.
