乡镇卫生院卫生人员工作压力、职业倦怠和离职意愿的关系研究

     

摘要

Objective To analyze the status of working pressure,job burnout and turnover intention among the staff in the township health centers,and to explore the relationship among the working pressure,job burnout and turnover intention,then put forward effective personnel incentives.Methods 45 THCs in Shandong,Anhui and Shanxi provinces health technicians were conducted to an on-site questionnaire investigation.Results The score of general work stress is 3.46±0.89.As for job burnout,the score of"Emotional exhaustion"is 19.13±10.43,"Depersonalization"is 3.92±4.30,and"Diminished personal accomplishment"is 11.49±8.59.As for turnover intention,the score of"Turnover Inclination"is 2.98±1.15,"Plan to search a new job"is 2.87±1.14,and"Possibility to get a new job"is 3.65±0.95.The results of structure equation model indicate that the path coefficient of working pressure to job burnout is 0.53,working pressure to turnover intention is 0.35,and job burnout to turnover intention is 0.21.So all of the path coefficient reached a significance level.The mediating effect of job burnout on the relationship between job pressure and turnover intention was 0.11,accounting for 23.9%of the total effect.Conclusion Our country should increase the government input and transfer payment,optimize the medical practice environment,improve the career development plan of the township health technical personnel,alleviate their working pressure and load,thus stabilize the health personnel team.%目的:分析乡镇卫生院卫生人员工作压力、职业倦怠和离职意愿状况,探索工作压力、职业倦怠和离职意愿的关系,从而提出有效的人员激励措施.方法:于2013年8月对山东、安徽和陕西三省45家乡镇卫生院卫生人员展开现场问卷调查.结果:总体工作压力得分为3.46±0.89.职业倦怠各维度得分为:情感耗竭19.13±10.43,去人性化3.92±4.30,低成就感11.49±8.59.离职意愿各维度得分为:产生离职想法2.98±1.15,寻找新的工作计划2.87±1.14,找到新工作的可能3.65±0.95.结构方程模型拟合结果表明,工作压力对职业倦怠的路径系数为0.53,工作压力对离职意愿的路径系数为0.35,职业倦怠对离职意愿的路径系数为0.21,路径系数均达到显著性水平,职业倦怠对工作压力和离职意愿关系的中介作用为0.11,占总效应的23.9%.结论:我国应加大政府投入及转移支付力度,优化医疗执业环境,做好乡镇卫生人员的职业发展规划,缓解其工作压力与负荷,从而稳定基层卫生人才队伍.
