HBV相关及HCV相关肝细胞肝癌组织中8-羟基脱氧鸟苷酸和CD34及Ki-67的表达研究

摘要

目的 比较HBV相关与HCV相关肝细胞肝癌(HCC)组织中8-羟基脱氧鸟苷酸(8-OHdG)、CD34及Ki-67的表达,分析三者与临床病理特征的关系.方法 选取HCC患者110例,其中HBV相关HCC 90例(B-HCC组),HCV相关HCC 20例(C-HCC组),患者均行手术治疗,术后病理证实诊断.对癌组织标本中8-OHdG、CD34、Ki-67进行免疫组织化学染色,半定量评分评价染色结果.比较B-HCC组和C-HCC组各标记物的表达情况.结果 B-HCC组、C-HCC组癌组织中8-OHdG阳性表达率均为100.0%,按阳性细胞百分比>25%分层比较,B-HCC组8-OHdG表达强(++~+++)者占67.8%(61/90),C-HCC组占90.0%(18/20),两组比较差异有统计学意义(P<0.05).B-HCC组8-OHdG的表达与有无门静脉瘤栓、临床分期有关(P<0.01),而C-HCC组8-OHdG的表达与各临床指标无关.B-HCC组CD34指数≥120者占8.9%(8/90),C-HCC组占35.0%(7/20),两组间差异有统计学意义(P<0.05).B-HCC组Ki-67阳性表达率为95.6%,C-HCC组为100.0%,按表达强度分层比较,两组Ki-67表达强度间差异无统计学意义(P>0.05).结论 在HBV相关及HCV相关HCC形成中,8-OHdG代表的氧化应激及CD34代表的血管密度参与作用的程度可能不同.%Objective To compare the expression of 8 - OHdG, CD34 and Ki - 67 protein, and clinicopathological features in two different groups of hepatocellular carcinoma ( HCC ), namely: hepatitis B virus related HCC and HCV related HCC. Methods Using immunohistochemical staining to determine the protein expression of 8 - OHdG, CD34 and Ki - 67 in the 110 cases including 90 HBV related HCC and 20 HCV related HCC. Results The expression of 8 - OHdG was observed in 90 out of 90 ( 100. 0% ) in HBV related HCC, and also in 20 out of 20 ( 100. 0% ) in HCV related HCC. In group of HBV related HCC, the patients with 8 - OHdG expression in >25% of malignant cells accounted for 67. 80% ( 61/90 ), in HCV related HCC was 90. 0% ( 18/20 ) . There was more in HCV related HCC than that of HCV related HCC ( P < 0. 05 ) . Significant correlation between the protein expression of 8 - OHdG with portal vein tumor embolus and clinical stage in B - HCC was found. The difference of patients with CD34 index more than 120 between two groups of HBV related HCC and HCV related HCC was statistical significance ( 8. 9% vs 35. 0% , P < 0. 05 ) . The positive rate of Ki - 67 expression in HBV related HCC was 95. 6% , also 100. 0% in HCV related HCC. But the difference was no significant in the two groups ( P > 0. 05 ) . Conclusion To some extent, 8 - OHdG expression representing stress oxidation and CD34 expression representing microvessel density may vary in hepatocarcinogenesis caused hy HBV and by HCV.
