超声弹性成像比值法与面积比法在鉴别乳腺良恶性肿块中的应用价值

摘要

Objective To investigate the application value of elastosonography ratio method and area ratio method in the identification of benign and malignant breast lumps. Methods 45 patients with breast lump diagnosed and given operation in the department of breast surgery of Peking University Peopleˊs Hospital from December 2012 to June 2013 were selected,and there were a total of 45 lesions. Conventional ultrasound and ultrasonic elastography were used and strain ratio and area ratio were calculated. Pathology diagnosis was taken as diagnosis standard. Results Among the 45 breast masses,17 were benign and 28 were malignant. The strain ratio and area ratio between benign and malignant masses showed statistically significant difference( P<0. 05 ) . The sensitivity, specificity, best cutoff point and area under ROC curve of elastosonography ratio method were 64. 3%,82. 4%,2. 78 and 0. 77 respectively. While the sensitivity,specificity,best cutoff point and area under ROC curve of elastosonography area ratio method were 46. 4%,94. 1%,1. 95 and 0. 70. Conclusion The ultrasonic elastography shows great clinical value in the diagnosis of malignant and benign breast masses.%目的:探讨超声弹性成像比值法与面积比法在乳腺良恶性肿块鉴别诊断中的应用价值。方法选取2012年12月-2013年6月在北京大学人民医院乳腺外科就诊并进行手术的乳腺肿块患者45例,共45个病灶,应用常规超声及超声弹性成像进行检查,计算应变率比值和面积比,以病理结果作为诊断标准。结果45个乳腺肿块中良性肿块17个,恶性肿块28个。良性和恶性乳腺肿块应变率比值和面积比比较,差异均有统计学意义( P<0.05)。弹性成像比值法诊断乳腺恶性肿块的灵敏度为64.3%,特异度为82.4%,最佳截点为2.78,受试者工作特征曲线下面积为0.77。弹性成像面积比法诊断乳腺恶性肿块的灵敏度为46.4%,特异度为94.1%,最佳截点为1.95,受试者工作特征曲线下面积为0.70。结论超声弹性成像技术在鉴别乳腺肿块良恶性方面有一定的临床诊断价值。
