糖尿病家族史与高尿酸血症对2型糖尿病发病的协同作用研究

摘要

[AbstraCt] ObjeCtive To explore the synergistic effect of diabetes family history and hyperuricemia on the incidence of type 2 diabetes. Methods In 2009,a stratified random sampling method was used to extract 6 000 community residents from the original 7 districts and 5 cities of Qingdao as the research subjects and eventually 5 165 residents finished investigation with a response rate of 86. 1%. By epidemiological investigation on the spot,791 newly diagnosed type 2 diabetes patients were chosen as the case group,and 4 374 people with normal blood glucose in the control group. An additive model was used to analyze the influence of the interaction between diabetes family history and hyperuricemia on incidence of type 2 diabetes. Results 145 patients(18. 3% )in case group had family history of diabetes and 458 cases in the control group(10. 5% )had family history of diabetes;the difference was statistically significant(χ2 = 40. 10,p < 0. 001). 145(18. 3% )in case group were patients with hyperuricemia and 612( 14. 0% ) in the control group were patients with hyperuricemia;the difference was statistically significant(χ2 = 10. 10,p = 0. 001). Univariate non _ conditional logistic analysis showed that family history of diabetes was the risk factor for type 2 diabetes(OR = 1. 862,p < 0. 001);hyperuricemia was the risk factor(OR = 1. 337,p = 0. 021);when family history and hyperuricemia co _ existing,the risk of type 2 diabetes increased(OR = 2. 885,p < 0. 001)and family history of diabetes and hyperuricemia had a positive interaction on the onset of type 2 diabetes. Multivariate logistic regression analysis showed that when the factor of family history of type 2 diabetes existed alone,OR value was 1. 781 ( p < 0. 001);when hyperuricemia existed alone,OR value was 1. 212(p = 0. 015);when they both existd,OR value was 2. 912(p < 0. 001);family history of diabetes and the incidence of hyperuricemia in type 2 diabetes were still positively interacted. After adjusting confounding factors by multiple logistic regression analysis,the synergy index was 2. 932,the attributable interation was 0. 919, the attributable interaction percent was 31. 56% and the pure attributable interaction percent was 48. 06%. ConClusion Diabetes family history and hyperuricemia had a synergistic effect on the incidence of type 2 diabetes.%目的:探讨糖尿病家族史与高尿酸血症在2型糖尿病(T2DM)发病中的协同作用。方法于2009年,采用分层随机抽样方法,在青岛的原七区五市共随机抽取6000名社区居民为研究对象。其中实际完成调查5165人,应答率为86.1%。将现场流行病学调查筛查出的791例新发2型糖尿病患者作为病例组,同期调查的4374名血糖正常者作为对照组,运用相加模型分析糖尿病家族史与高尿酸血症的交互作用对2型糖尿病发病的影响。结果病例组中有145例(18.3%)有糖尿病家族史,对照组中有458例(10.5%)有糖尿病家族史,两组间差异有统计学意义(χ2=40.10,p <0.001)。病例组中有145例(18.3%)为高尿酸血症患者,对照组中有612人(14.0%)为高尿酸血症患者,两组间差异亦有统计学意义(χ2=10.10,p =0.001)。单因素非条件 Logistic 回归分析结果显示,糖尿病家族史是2型糖尿病发病的危险因素( OR =1.862,p <0.001);高尿酸血症也是其危险因素( OR =1.337,p =0.021);糖尿病家族史与高尿酸血症共同存在时,2型糖尿病的发病危险增加(OR =2.885,p <0.001),糖尿病家族史与高尿酸血症对2型糖尿病的发病有正交互作用。多因素非条件 Logistic 回归分析结果显示,糖尿病家族史单独存在时对2型糖尿病发病的 OR 值为1.781(p <0.001);高尿酸血症单独存在时的 OR 值为1.212(p =0.015);两者共同存在时的 OR 值为2.912(p <0.001),糖尿病家族史与高尿酸血症对2型糖尿病的发病仍存在正交互作用。采用Logistic 回归调整混杂因素后,糖尿病家族史与高尿酸血症的协同效应指数为2.932,归因交互效应为0.919,归因交互效应百分比为31.56%,纯因子间归因交互效应百分比为48.06%。结论糖尿病家族史与高尿酸血症对2型糖尿病的发病有协同作用。
