目的：探讨皮肤软组织扩张在头面颈皮肤缺损修复中的应用效果。方法：选取2010年4月-2013年2月在笔者所在医院治疗的头面颈皮肤损伤患者50例为研究对象，所有患者均实施皮肤软组织扩张手术修复面颈部皮肤损伤，积极改善患者全身情况、创面处理，待病情稳定后实施一期创面封闭，置入扩张器，并扩张注水后行二期扩张皮瓣修复工作，观察治疗期间患者感染情况和扩张器外露情况并对手术修复缺损效果进行评价。结果：50例患者除2例患者因扩张囊外露导致失败选择其他修复方式进行治疗，其余48例均顺利治愈面颈部缺损，修复效果好；治疗期间并发症3例，分别为口角牵拉移位、瘢痕性秃发、轻度眼睑外翻，对症治疗后好转康复。结论：皮肤软组织扩张在头面颈皮肤缺损修复中的应用效果较好，但是要根据患者病情进行合理选择与正确应用，以达到最佳效果，适宜临床大力应用推广。%Objective:To investigate the application of the skin and soft tissue expansion in the head,face and neck skin repair.Method:50 cases with head,face and neck skin lesions in our hospital from April 2010 to February 2013 were selected.All patients were implemented with skin and soft tissue expansion surgery to repair skin damage.They were actively improved with regard to the general condition and wound treatment.When the condition was stable,the wound was closed and placed with a dilator,with expanding water,followed by two expanded flap repairing.then to observe the infection condition and dilator exposure during treatment to evaluate the effect of the repairing surgery.Result:50 cases with face and neck defects were cured except for 2 cases.The repair turned out to be a success.Two cases failed because of the exposed expander and took other treatment.Complications during treatment were pulling altercation shift,scarring alopecia, mild eversion of the lid.Symptoms were improved after corresponding treatment.Conclusion:Skin and soft tissue expansion in the head,face and neck skin defects woks well,but in order to get the best outcome,the application should be based on fully analysis about the patient's condition.The surgery is suitable for clinical and further application.
