Purpose: To investigate the value of elastography and contrast enhanced ultrasound in predicting cervical lymph node metastases of papillary thyroid carcinoma (PTC). Methods: Two hundred and thirty-eight cases of PTC confirmed by pathology results were enrolled in this study. All the patients were performed elastography and contrast enhanced ultrasound before surgery, and the relationship between lymph node metastases and the characteristics on elastography and contrast enhanced ultrasound was investigated. Results: Univariate analysis showed that there were statistically significant differences in lymph node metastases of PTC with Age> 45 years, size> 8.95 mm, unclear boundary, irregular shape, capsular invasion and rear sound shadow. Multivariate analysis showed that there were statistical differences in age, size, capsular invasion and elastic score on lymph node metastases of PTC (P < 0.05). Conclusion: Elastography and contrast-enhanced ultrasound have certain predictive value for lymph node metastases of PTC, which is helpful for clinical diagnosis and treatment.%目的:探讨甲状腺乳头状癌 (PTC) 的超声弹性成像及超声造影对判断颈部淋巴结转移的价值.方法:选取均行超声造影和弹性成像并经手术证实为PTC的患者238例, 回顾性分析各项指标与PTC淋巴结转移的关系.结果:①单变量分析发现年龄> 45岁、> 8.95mm、边界不清、形态不规则、侵犯包膜、后方有声影、超声造影灌注不均匀、弹性成像评分> 4分均对PTC淋巴结转移有统计学差异 (P均<0.05).②多变量分析发现年龄、大小、侵犯包膜、弹性评分对PTC淋巴结转移有统计学差异 (P均<0.05).结论:超声弹性成像及超声造影对PTC淋巴结转移有一定的预测价值, 有助于临床诊断及治疗.
