血清HE4、CA125、CA15-3含量对卵巢癌诊断及预后评估的临床价值分析

Objective To analyze clinical value by serum human epididymis protein 4 (HE4), carbohydrate antigen 125 (CA125), carbohydrate antigen 15-3 (CA15-3) for diagnosis and prognosis evaluation of ovarian cancer. Methods There were 55 patients with ovarian cancer as group A, 66 patients with benign pelvic disease as group B, and 45 healthy women as control group. Analysis and comparison were made on serum HE4, CA125 and CA15-3 levels in all groups, along with preoperative and postoperative serum HE4, CA125 and CA15-3 levels in ovarian cancer patients. Results Group A had obviously higher HE4, CA125 and CA15-3 levels than group B and control group (P<0.05), and group B had much higher CA125 and CA15-3 levels than control group (P<0.05). Combined detection of HE4+CA125+CA15-3 showed higher sensitivity as 92.73%, specificity as 90.99%, positive predictive value as 83.61% and negative predictive value as 96.19% than single index detection. There were 36 patients received surgical treatment, and they had all lower HE4, CA125 and CA15-3 levels after operation than those before operation (P<0.05). Conclusion As a ovarian tumor marker with high specificity, HE4 combining with CA125 and CA15-3 in detection can improve diagnostic accordance rate of ovarian cancer. This method contains highly clinical value for prognosis evaluation of ovarian cancer.%目的：分析血清人附睾上皮分泌蛋白4(HE4)、糖类抗原125(CA125)、糖类抗原15-3(CA15-3)含量对卵巢癌诊断以及患者治疗预后评估的临床价值。方法55例卵巢癌患者作为 A 组,66例盆腔良性疾病患者作为 B 组,45例健康女性作为对照组,分析比较三组血清 HE4、CA125、CA15-3水平,以及卵巢癌患者手术前后 HE4、CA125、CA15-3水平。结果A 组 HE4、CA125、CA15-3水平显著高于B 组及对照组(P<0.05)；B 组 CA125、CA15-3水平明显高于对照组(P<0.05)。HE4+CA125+CA15-3联合检测的敏感性为92.73%、特异性为90.99%、阳性预测值为83.61%、阴性预测值为96.19%,均高于单一指标的检测方法。本次36例卵巢癌患者接受手术治疗,患者手术后 HE4、CA125、CA15-3水平均明显低于手术前(P<0.05)。结论HE4是一种特异性较好的卵巢肿瘤标志物,与 CA125、CA15-3联合检测能提高卵巢癌的诊断符合率,对于卵巢癌的预后评估也有较高的临床价值。
