根本原因分析法用于降低药品调剂差错的实践

摘要

目的：分析药品差错，制订标准工作流程，改善工作环境和不良习惯，降低药品调剂差错，确保患者用药安全。方法采用根本原因分析法的基本原理及鱼骨图找出药品调剂差错的原因。结果与结论药师按规定调剂药品是防范用药差错的关键环节。药师在调剂药品时应严格落实审方要求，完善和推进药师的药学服务，以更好地保障患者的用药安全。%Objective To analyze the medication errors,set standard work processes,improve working environment and bad habits,reduce medicine dispensing errors,and ensure patients medication safety.Methods The basic principle of Root Cause Analysis and its tools of Fishbone Diagram was used to find out the cause of medicine dispensing errors.Results and Conclusion Dispensing medicines as re-quired for pharmacists is the most critical to prevent medication errors.When dispensing medicines,pharmacists should strictly implement the requirements for prescription examination,improve and promote their pharmacy services,and ensure patients medication safety better.
