静脉序贯及口服补铁联合促红细胞生成素治疗肾性贫血的效果分析

摘要

目的：探讨静脉序贯及口服补铁联合促红细胞生成素治疗肾性贫血的临床效果。方法选择2013年7月~2015年1月我院收治的肾性贫血患者80例，分为两组，各40例。两组均使用促红细胞生成素静脉推注，对照组常规口服琥珀酸亚铁，观察组治疗过程的第1周使用蔗糖铁静脉滴注，2次/周，100 mg/次，每次滴注时间维持0.5 h，1周后改为琥珀酸亚铁口服补充。两组均连续治疗12周为1个疗程。结果治疗后，观察组的血清铁蛋白、转铁蛋白饱和度、血红蛋白、血细胞比容均高于治疗前及对照组治疗后（P<0.05）；观察组血红蛋白每周上升速度显著快于对照组，达到靶治疗目标时间显著短于对照组（P<0.05）；观察组发生食欲减退和腹泻腹胀的比例显著低于对照组（P<0.05）。结论静脉序贯及口服补铁联合促红细胞生成素治疗肾性贫血临床起效时间增快，治疗有效性提高，不良反应少。%Objective To explore the clinical effect of sequential introvenous and oral supplement iron combined with erythropoietin treating renal anemia. Methods 80 patients with renal anemia from July 2013 to January 2015 of our hospital were selected.Patients were divided into two groups,and each group had 40 patients.Both groups were given erythropoietin for intravenous injection.Control group was given oral ferrous succinate routinely.In the first week,obser-vation group was given sucrose iron for intravenous drip,two times per week,100 mg one time.The drip time maintained half an hour once time.One week later,oral supplement ferrous succinate was used.Two groups were treated for 12 weeks (one course). Results After treatment,serum ferritin,transferrin saturation,hemoglobin,hematocrit in observation group was higher than that before treatment and control group after treatment (P<0.05).The rising velocity in every week of hemoglobin in observation group was significantly faster than that of control group,and the time to get the target treatment goal was significantly shorter than that of control group (P<0.05).The proportion of appetite decreases,diarrhea and abdominal distention in observation group was significantly lower than that of control group (P<0.05). Conclusion Sequential introvenous and oral supplement iron combined with erythropoietin treating renal anemia can accelerate the clinical effect onset time,enhance the effectiveness of therapy and has little adverse reaction.
