紫绀型先天性心脏病患儿的急诊手术治疗经验

摘要

Objective To summarize the experience of emergency surgical treatment for cyanotic congenital heart disease(CHD) in children.Methods Clinical data of emergency operations in 89 children [64 males and 25 females,(4 ± 3) months] with cyanotic CHD were retrospectively reviewed from January 2016 to August 2017 in Beijing Anzhen Hospital,Capital Medical University.Corrective operations were performed in 83 cases and palliative operations were performed in 6 cases.Radical cure rate,postoperative complications and prognosis of children were analyzed.Results Radical cure rate was 93.3％ (83/89).Length of hospital stay was 8-40 days,with an average of (18 ± 9) days.Incidence of postoperative complications was 25.8％ (23/89).Early postoperative mortality rate was 3.4％ (3/89).Conclusion It is necessary for children with cyanotic CHD to have emergency surgical treatment after diagnosis,and reasonable perioperative management is an important guarantee for the prognosis of children.%目的 总结小儿紫绀型先天性心脏病(CHD)急诊手术经验.方法 回顾性分析2016年1月至2017年8月首都医科大学附属北京安贞医院小儿心脏中心急诊手术治疗的89例紫绀型CHD患儿的临床资料.其中男64例、女25例,年龄(4±3)个月.行外科根治手术83例,姑息手术6例.统计分析手术根治率、术后并发症及患儿预后.结果 本组89例手术患儿心脏畸形一期根治率为93.3％ (83/89),住院时间8 ～40 d、平均(18 ±9)d.术后并发症发生率25.8％ (23/89).全组术后早期死亡3例,病死率为3.4％(3/89).结论 紫绀型CHD患儿明确诊断后需尽快行急诊手术治疗,合理的围术期管理是患儿良好预后的重要保障.
