目的：探讨髌骨适形加压接骨板配合加压骨栓治疗老年髌骨非粉碎性骨折的临床效果。方法选取2012年2月～2013年8月东莞市虎门中医院和东莞市第五人民医院收治的68例老年髌骨非粉碎性骨折患者随机分为观察组和对照组，对照组采用克氏针张力带钢丝固定术治疗，观察组采用髌骨适形加接骨板配合加压骨栓治疗，比较两组患者手术时间、术中出血量及临床骨折愈合时间，对两组患者随访8个月统计两组患者并发症发生情况及膝关节功能恢复情况。结果观察组手术时间和术中出血量均少于对照组，但两组比较无统计学意义（P＞0.05）；观察组临床骨折愈合时间明显短于对照组，两组比较有统计学意义（P＜0.05）；观察组术后8个月膝关节功能优良率为100.00％明显高于对照组88.24％的优良率，两组比较有统计学意义（P＜0.05）；观察组并发症发生率为2.94％明显少于对照组14.71％的发生率，两组比较差异有统计学意义（P＜0.05）。结论髌骨适形加压接骨板配合加压骨板治疗老年非粉碎性骨折具有显著固定效果，可促进骨折愈合、减少并发症发生，提高膝关节功能恢复，具有显著临床治疗效果，值得在临床中推广应用。%ObjectiveTo explore the clinical effect of the patellar conformal compression plate combined with compression bolt in the treatment of senile non-comminuted fractures of patella.Methods 68 elderly patients with non-comminuted patella fracture who admitted in our hospital from 2012 February to 2013 August were selected and randomly divided into the observation group and the control group. The control group were treated using Kirschner wire and tension band wire fixation, while the observation group were treated with patellar conformal compression plate combined with compression bolt, and then operation time, the amount of blood loss in operation and clinical healing time of fracture of two groups were compared. However, all the patients were followed up for 8 months, and then, complications and the recovery of knee joint function of two groups were statistical and analyzed.ResultsThe operation time and bleeding of the patients in the observation group were significantly less than those of the patients in the control group, but there were no statistically significant difference between the two groups (P>0.05); the clinical healing time of fracture of patients in observation group was significantly shorter than that of the patients in the control group, the difference between two groups was statistically significant (P<0.05); the excellent and good rate of knee joint function of the patients in the observation group was 100.00% at 8 months after operation, which was significantly higher than that of patients in the control group (88.24%), the difference between two groups was statistically significant (P<0.05); the incidence of complication of observation group was 2.94%, significantly lower than that of the control group, which was 14.71%, the difference between two groups was statistically significant (P<0.05).Conclusion patellar conformal compression plate combined with compression bolt has significant fixed effect on the senile non-comminuted fracture of patella, can promote the healing of fracture, reduce the incidence of the complications, and can improve the recovery of function of knee joint, which has significant clinical therapeutic effect, and was worthy of promotion in clinical application.
