三维数字减影血管造影下介入手术对颅内复杂动脉瘤的疗效分析

         

摘要

目的:探讨三维数字减影血管造影(3D-DSA)引导下介入治疗颅内动脉瘤的应用价值.方法:选取医院收治的128例颅内动脉瘤患者,将其以数表法随机分为观察组及对照组,每组64例.对照组患者给予临床常规开颅手术治疗,观察组患者给予3D-DSA引导下介入手术治疗,对比分析两组患者治疗效果、手术情况、并发症产生情况以及患者满意程度等指标.结果:观察组患者治疗总有效率为87.5%,对照组总有效率为68.8%,两组比较观察组明显优于对照组,差异具有统计学意义(x2=5.233,P<0.05);观察组患者手术时间为(54.3±3.3)min明显少于对照组的(67.2±5.3)min,差异具有统计学意义(t=4.283,P<0.05);观察组患者术中出血量为(42.3±3.2)ml明显少于对照组的(313.7±42.8)ml,差异具有统计学意义(t=16.293,P<0.05);治疗后观察组患者并发症产生情况明显少于对照组患者,差异具有统计学意义(x2=6.392,P<0.05);术后对患者及其家属进行随访,观察组患者满意程度明显高于对照组患者,差异具有统计学意义(x2=8.392,P<0.05).结论:3D-DSA引导下介入手术治疗颅内动脉瘤有效率高、并发症少,同时可极大提升患者对治疗的满意度,具有较高的临床应用价值.%Objective:To explore the application value of interventional therapy under three dimensional-digital subtraction angiography (3D-DSA) in clinical treatment for intracranial complex aneurysms.Methods: 128 patients with intracranial aneurysms were divided into observation group (64cases) and control group (64cases) as random number table. The patients of control group were treated by conventional craniotomy, while the patients of observation group were treated by interventional therapy under 3D-DSA. The therapeutic effects, situations of surgery, situations of complication and satisfied degrees of patients of the two groups were compared and analyzed. Results: The total effective rate of observation group (87.5%) was significantly higher than that of control group (68.8%)(x2=5.233,P<0.05). The operating time of observation group (54.3±3.3) min was significant less than that of control group (67.2±5.3)min (t=4.283,P<0.05). The bleeding volume of observation group (42.3±3.2) ml was significant lower than that of control group (313.7±42.8ml) (t=16.293,P<0.05). After therapy, the complications of observation group were significant less than that of control group (x2=6.392,P<0.05). The patients and their family member were followed up post-operation, and the satisfaction degrees of observation group were significantly higher than that of control group (x2=8.392,P<0.05).Conclusion: The intervention therapy under 3D-DSA for intracranial aneurysms has high efficiency and less complications, at the same time, it can extremely improve the satisfaction degrees of patients. Therefore, it has higher application value in clinical practice.
