经脐单孔腹腔镜手术治疗良性卵巢肿瘤的疗效观察

摘要

目的 探讨单孔腹腔镜手术在卵巢良性肿瘤治疗方面的临床应用价值.方法 将2014年2月-2016年8月收治的卵巢良性肿瘤患者分为两组,每组41例,观察组给予单孔腹腔镜手术治疗,对照组给予传统多孔腹腔镜妇科手术治疗.观察并记录两组患者手术效果、术中与术后并发症情况、术后卵巢功能及美容满意度.结果 两组患者手术过程顺利,无术中与术后并发症.观察组手术时间和术后美容满意度评分明显高于对照组(P <0.05),观察组术后排气时间、术后住院时间、术后24与72 h VAS评分明显低于对照组(P <0.05);两组术后雌二醇水平较术前明显降低(P <0.05),术后卵泡刺激素和黄体生成素水平较术前明显升高(P <0.05);两组术中出血量、手术前后雌二醇、卵泡刺激素和黄体生成素水平无明显差异(P >0.05).结论 单孔腹腔镜技术用于治疗良性卵巢肿瘤较多孔腹腔镜技术效果更好且无并发症,值得推广应用.%Objective To investigate the clinical value of single hole laparoscopy in treatment of ovarian benign neoplasms. Methods The patients with benign ovarian tumors treated from February 2014 to August 2016 were divided into two groups, 41 cases in each. The observation group received laparoendoscopic single-site surgery, while the control group was treated with traditional porous laparoscopic gynecologic surgery. The operation effect, intraoperative and postoperative complications, postoperative ovarian function and cosmetic satisfaction were observed and recorded in the two groups. Results The operation of the two groups was smooth and there were no intraoperative or postoperative complications. The operation time and postoperative satisfaction degree of the observation group were significantly higher than that of the control group (P < 0.05). The postoperative exhaust time, postoperative hospital stay, postoperative 24 h and 72 h, VAS scores of the observation group were significantly lower than those of the control group (P < 0.05). The estradiol levels in the two groups were significantly lower than those before operation, and the levels of follicule-stimulating hormone and luteinizing hormone were significantly higher than those before operation (P < 0.05). There were no significant differences in the amount of bleeding between the two groups and the levels of estradiol, follicule-stimulating hormone and luteinizing hormone before and after the operation (P > 0.05). Conclusion The laparoendoscopic single-site surgery technique is better than the porous laparoscopic technique in the treatment of benign ovarian tumors, and it is worthy of popularization and application.
