128层螺旋CT冠状动脉成像在婴幼儿圆锥动脉干畸形的应用

     

摘要

目的:比较前瞻性心电门控与回顾性心电门控冠状动脉CT对于小儿先天性心脏病的诊断价值、图像质量及辐射剂量.方法:回顾性分析76例经手术证实的先天性心脏病,所有患者均行CTA扫描,并记录患者心率、扫描时间、扫描剂量.其中行前瞻性心电门控及回顾性心电门控冠状动脉CT各38例,76例患者均经手术证实并在术前行经胸骨后心脏超声.所有CTA图像由两名影像科医师对图像质量进行判断,判断标准主要为是否能够观察心内结构、心外大血管、冠状动脉.结果:76例患儿均成功完成扫描,76例患者的心内结构、心外大血管及冠状动脉均能清晰显示,其中前瞻心电门控患者的扫描剂量及时间与回顾性心电门控相比有显著差异(P<0.05).结论:前瞻性心电门控与回顾性心电门控冠状动脉成像均能用于诊断少儿圆锥动脉干畸形,其中前瞻性心电门控在保证图像质量的前提下扫描剂量明显低于回顾性心电门控冠脉成像.%Objective: To compare image quality, dose, diagnostic value of perspective ECG-triggering and restrospective ECG gating coronary artery computed tomography. Methods: Seventy six patients with conotruncal defects and underwent surgery were collected in our study, heart rate, scanning time, radiation dose of the patients were obtained and thirty eight of whom were performed perspective ECG-triggering while the rest performed retrospective ECG gating coronary artery computed tomography. Also all the seventy-six patients were performed transthoracic echocardiography. Two radiologists assessed all the images of computed tomography scanning, including structure of heart, vessels and coronary artery. Results: The structure of heart, vessels and coronary artery of all seventy six patients were clearly display and there were a significant difference between perspective ECG-triggering and retrospective ECG gating coronary artery computed tomography. Conclusions: Both perspective ECG-triggering and retrospective ECG gating coronary artery computed tomography have the ability in diagnosing Conotruncal defects, and perspective ECG-triggering computed tomography have reduced radiation doses than retrospective ECG gating coronary artery computed tomography with equivalent image quality.
