Objective To evaluate the clinical efficacy of lens loop nucleus - deliver and lens loop - pad nucleus chopping in the small incision extracapsular hard nucleus cataract extraction. Methods All 280 hard nucleus cataract patients from cataract operation were chosen and randomized into lens loop nucleus - deliver group ( group A ) including 140 patients ( 140 eyes ) and lens loop - pad nucleus chopping group ( group B ) including 140 patients ( 140 eyes ). Postoperative visual acuity, corneal astigmatism and complications were compared in two groups. Results Postoperative best - corrected visual acuity in group A was lower than that in group B, hut corneal astigmatism was higher than that in group B( t = -4. 315 P =0. 001 , t = 5. 313 P =0. 000 ), and there was significant difference ( P ＜0.05 ); when two groups were compared 3 months after operation ( t = -0. 814 P = 0.414 ), there was no significant difference. Posterior capsule rupture, iris injury and leakage of incision in group A was higher than in group B ( X2 = 5. 937 P = 0. 015, X2 =6.629 P =0. 01, X2 =6. 131 P = 0. 013 ), and there was significant difference. Conclusion Both lens loop nucleus - deliver and lens loop - pad nucleus chopping can have good effect on the small incision extracapsular hard nucleus cataract extraction; lens loop - pad nucleus chopping has the advantage of rapid recovery and less complication, and is worth popularizing in primary hospitals.%目的 探讨圈套式娩核法和圈垫式劈核法在小切口硬核白内障囊外摘出术临床应用效果.方法 选取白内障复明手术中280例硬核白内障患者,随机分成圈套式娩核法组(A组)140例(140眼)和圈垫式劈核法组(B组)140例(140眼),比较两组的术后视力和角膜散光度以及并发症发生情况.结果 A组术后近期矫正视力低于B组,而角膜散光度高于B组(t=-4.315,P=0.001;t=5.313,P=0.000),差异有统计学意义(P<0.05),术后3个月两组比较(t=-0.814,P=0.414)差异无统计学意义.A组后囊破裂、虹膜损伤、切口渗漏发生率高于B组(χ2=5.937,P=0.015;χ2=6.629,P=0.01;χ2=6.131,P=0.013),差异有统计学意义.结论 圈套式娩核法和圈垫式劈核法在小切口硬核白内障囊外摘出术均能获得良好复明效果,圈垫式劈核法具有视力恢复快、并发症少的优势,适合在基层医院复明手术中应用.
