针刺对结肠吻合术后大鼠结肠Cajal间质细胞受体酪氨酸激酶c-kit表达和结肠组织一氧化碳及血红素氧合酶的影响

         

摘要

目的:观察针刺对结肠吻合术后结肠Cajal间质细胞(ICCs)受体酪氨酸激酶c-kit表达和结肠组织一氧化碳(CO)及血红素氧合酶(HO)的影响,探讨针刺改善胃肠动力的机制.方法:SD大鼠随机分为空白组、模型组和针刺组,采用结肠吻合术造模,每组分术后3、5、10 d 3个时段,共9个亚组,每亚组10只.针刺组于术后次日予针刺干预,取双侧"足三里""三阴交""太冲"穴,留针15 min,1次/d.记录术后首次排便时间,计算各时段小肠推进率,采用免疫组化法检测结肠c-kit表达,用生化法检测结肠组织CO含量,ELISA法检测结肠组织HO-1、HO-2活性.结果:与空白组比较,模型组小肠推进率下降(P<0.05);c-kit表达水平减少(P<0.05);术后3、5 d 结肠组织CO含量和HO-1活性升高(P<0.05),后恢复正常;术后3 d结肠组织HO-2活性升高(P<0.05),而术后5、10 d HO-2活性降至低于空白组(P<0.05).与模型组比较,针刺组术后首次排便时间缩短(P<0.05);小肠推进率升高(P<0.05);c-kit表达水平增加(P<0.05);术后3、5 d 结肠组织CO含量和HO-1活性降低(P<0.05);术后3 d结肠组织HO-2活性降低(P<0.05),而术后10 d HO-2活性升高(P<0.05).与术后3 d比较,模型组及针刺组术后5 d亚组小肠推进率升高,c-kit表达水平增加,结肠组织CO含量和HO-1、HO-2活性均降低(均P<0.05);与术后5 d比较,模型组术后10 d亚组小肠推进率升高,c-kit表达水平增加,结肠组织CO含量和HO-1活性均降低(均P<0.05).结论:针刺能降低结肠组织CO含量和HO-1、HO-2活性,升高c-kit表达水平,促进术后ICCs修复,进而改善术后胃肠动力.%Objective To observe the effect of acupuncture on expression of receptor tyrosine kinase (RTK) c-kit in interstitial cells of Cajal (ICCs), carbon monoxide (CO) and heme oxygenase (HO) in colon tissue after colonic anastomosis, so as to explore the mechanism of acupuncture in improving gastrointestinal motility.Methods SD rats were randomly devided into control group, model group and acupuncture group.The model was established using colonic anastomosis.Each group was further devided into 3, 5, and 10 d (time-point) subgroups (n=10 in each).Acupuncture was applied to acupuncture group at bilateral "Zusanli" (ST 36), "Sanyinjiao" (SP 6) and "Taichong" (LR 3) for 15 min, once daily after modeling.The first defecation time was recorded, and the intestinal propulsive rate was measured.The expression of c-kit in colon tissue was detected by immunohistochemistry.The content of CO, the activities of HO-1 and HO-2 in colon tissue were detected by biochemical method and ELISA, respectively.Results Compared to the control group, the intestinal propulsive rate and the expression level of c-kit in ICCs were decreased in the model group(P<0.05), the content of CO and the activity of HO-1 were increased in model 3 d and 5 d subgroups(P<0.05), the activity of HO-2 was increased in model 3 d subgroup(P<0.05), while the opposite results appeared in model 5 d and 10 d subgroups(P<0.05).Compared to the model group, the first defecation time was shortened (P<0.05), the intestinal propulsive rate and the expression level of c-kit in ICCs were increased in the acupuncture group (P<0.05), the content of CO and the activity of HO-1 were decreased in acupuncture 3 d and 5 d subgroups(P<0.05), the activity of HO-2 was decreased in acupuncture 3 d subgroup(P<0.05), while the opposite result appeared in acupuncture 10 d subgroup(P<0.05).Compared to the 3 d subgroup, the intestinal propulsive rate and the expression level of c-kit in ICCs were increased, the content of CO, the activities of HO-1 and HO-2 were decreased in both model and acupuncture 5 d subgroups (P<0.05).In model 10 d subgroup, the intestinal propulsive rate and the expression level of c-kit in ICCs were increased, the content of CO and the activity of HO-1 were decreased in comparison with the model 5 d subgroup(P<0.05).Conclusion Acupuncture can improve postoperative gastrointestinal motility by declining CO content and HO-1 and HO-2 activity in colon tissue, and promoting ICCs restoration.
