新疆维吾尔族HLA-DQB1基因多态性与HBV感染结局的相关性

         

摘要

目的 研究人类白细胞抗原-DQB1(HLA-DQB1)基因多态性与新疆维吾尔族人群乙型肝炎病毒(HBV)感染结局以及HBV不同复制状态的相关性.方法 临床收集新疆维吾尔族慢性乙型肝炎(CHB)110例,记为CHB组;慢性HBV携带(ASC)100例,记为ASC组;自限性HBV感染(RHBS)80例,记为RHBS组;健康献血者45例,记为正常组.采用序列特异性引物-聚合酶链反应(PCR-SSP)法检测HLA-DQB1基因型,比较组间基因频率差异;比较HBV不同复制状态下HLA-DQB1基因分布频率.结果 DQB10201在RHBS组分布频率高于ASC组(18.75% vs 10.50%,χ2=5.959,P<0.05,OR=2.257);DQB10201在CHB组分布频率高于ASC组(17.73% vs 10.50%,χ2=5.363,P<0.05,OR=2.066);DQB10301在CHB组分布频率高于ASC组(26.82% vs 16.50%,χ2=9.062,P<0.01,OR=2.349);DQB10303在CHB组分布频率低于ASC组(19.55% vs 31.00%,χ2=10.996,P<0.01,OR=0.393);其他各组间等位基因频率差异无统计学意义;DQB10201在低复制组分布频率高于高复制组(17.08% vs 10.56%,χ2=4.295,P<0.05,OR=1.939).结论 HLA-DQB10201是新疆维吾尔族HBV抗性基因,DQB10301与HBV的持续感染有关,DQB10303是ASC易感基因.%Objective To invest the associations of human leukocyte antigen-DQB1 polymorphism with the outcome of HBV infection and the replication of HBV in Uygur population of Xinjiang. Methods 110 chronic hepatitis B patients, 100 asymptomatic HBV carriers,80 patients with self-cimited HBV infection and 45 healthy individuals were included in our study. DQB1 genotyping was conducted with polymerase chain reaction-sequence specific primer( PCR-SSP ). The frequency distributions of genotype were analyzed, and the frequency of that in different replication of HBV was studied. Results ①the frequency of the DQB1 * 0201 allele in RHBC group was significantly higher than ASC group( 18. 75% vs 10. 50% ,χ2=5.959,P <0. 05,OR =2. 257 );② the frequency of the DQB1 * 0201 allele was significantly higher in the CHB group than in the ASC group( 17. 73% vs 10. 50% ,χ2= 5. 363 ,P <0. 05 , OR = 2. 066 ); ③the frequency of the DQB1 * 0301 allele in CHB group was significantly higher than ASC group( 26. 82% vs 16. 50% ,χ2 = 9. 062, P <0. 01 , OR = 2. 349 ); ④the frequency of DQB1 * 0303 allele in CHB group was significantly lower than ASC group( 19. 55% vs 31. 00% , χ2= 10.996, P < 0. 01 , OR = 0. 393 ); ⑤ the frequency of the DQB1 * 0201 allele had significant difference in high replication and low replica-tion( 17. 08% vs 10. 56% , χ2= 4.295 , P < 0. 05 , OR = 1. 939 ). Conclusion Alleles of HLA-DRB1 * 0201 are closely associated with HBV clearance, and DQB1 * 0301 allele could aggravate persistent infection of HBV. DQB1 * 0303 allele determines the susceptibility to ASC.
