Quantitative measurement of serum monoclonal component (MC) is one of the most important and valuable tools for diagnosis and monitoring disease activity in monoclonal gammopathies. We have developed a standardized procedure to quantify the MC in compliance with recommendations from the International Myeloma Working Group, which is included in the scope of laboratory accreditation (Exp 619/LE1320) by the "Entidad Nacional de Acreditacion" according to the International Standard ISO 15189 (2007) "Medical laboratories. Particular requirements for quality and competence" The procedure includes Capillary electrophoresis of serum samples on a Paragon CZE~R 2000, Albumin quantification of serum samples by Nephelometry and direct absorbance quantification at 214 nm of MC and Albumin percentage. The MC concentration is calculated using the area under the spike from the absorbance baseline of the MC and of the Albumin spikes related to serum Albumin concentration determined by Nephelometry as reference.
