Oligoelonal IgM bands in cerebrospinal fluid of neuromyelitis optioa spectrum disorders

Background: Neuromyelitis optica (NMO) is produced by humoral immune response. Patological studies have demonstrated a pronnunced deposit of IgM in perivascular spaces in the early lesions of NMO. Our hypothesis is that NMO-patients could have oligoclonal IgM bands (OCBM) in the cerebrospinal fluid (CSF). Objective: To describe the presence of Oligoclonal IgG bands (OCGB) and OCMB in the CSF of patients with NMO. Methods: Nine patients diagnosed of NMO and NMO spectrum disorders, fulfilling Wingerchuk criteria, and twelve patients diagnosed of definitive MS according McDonald criteria, have been studied. Paired serum and CSF samples analysis with OCGB and OCMB determination have been performed by immunodetection and isoelectrofocusing methods, respectively. Results: In NMO patients and NMO spectrum, OCGB were present in 2 cases (22.2%), and OCMB in 7 cases (77.7%). In MS patients, we found that OCGB were present in all cases (100%) and OCMB were present in 4 patients (33.3%). Pattern of CSF characterized by OCGB absence and OCMB presence is only observed in NMO. Conclusion: OCMB and OCGM determination in NMO and NMO spectrum disorders can help us to diagnose and to understand pathogenic mechanism of these diseases.
