Melt derived bioactive glass (Bioglass~direct R) has been used clinically since 1985. The ability of this material to form a bon with bone and soft tissue has been related to a series of surface reactions that occur when the material is subjected to an aqueous environment. The formation of a high-specific surface area silica layer and susequent precipitation of hydroxy-carbonate apatite are the critical steps that lead to this bonding layer. Recently, the sol-gel process has been used to create a series of bioactive glasses with an initial surface area greater than 125 m~2/g. These materials have demonstrated the ability to enhance bone regeneration and are absorbed in the body. Long term tests are underway to validate the utility of these glasses as bone graft substitutes.
