Sequential breakdown of myocardial proteins is characteristic of both increasing periods of ischemia and increased duration of reperfusion. Endogenous peptides are the first indicators of myocardial ischemia, released after 15 minutes of ischemia. Low mass (<50kDa) myocardial proteins are released from pre-necrotic cardiomyocytes, but only following extended periods of reperfusion. Myocardial proteins are detected immediately with necrotic cardiomyocytes.
