Introduction: Computer-assisted navigation systems for total knee surgery have become ever more popular in the last years. Much literature has substantiated the benefits of these systems for aligning the implants. In order to speed up recovery time, muscle-sparing surgical techniques have been proposed in the last years, in particular the subvastus approach for TKR. The goal of this study is to verify the additional benefits of computer assistance when combined with a restricted incision size compared to the conventional subvastus approach. Methods: Two evaluations were conducted sequentially. First, 50 patients were operated using a conventional subvastus approach; they were randomized to be operated with or without computer assistance. Navigated cases were performed with the NK-n specific software VERSION 1.1 (OrthoSoft, Montreal). Secondly, 70 patients were operated using a subvastus approach without patella eversion; they were randomized with or without computer assistance. Navigated cases were performed with the OrthoSoft Universal TKR VERSION 2.0 (OrthoSoft, Montreal).
