Blood transfusion is one of the routes for transmission of disease. Prevention of such transmission depends primarily on structure and organisation of the blood supply from vein to vein, the human resources involved and their education and training as well as their authority, responsibility and accountability. Above all it depends on the implementation and functioning of a quality system and quality management. Key elements are leadership at any level in the organisation, human resources and quality awareness. Since the United Nations in 1948 formally declared in writing the human rights [1] with health, education and shelter for all as the elementary factors, a number of initiatives has been developed such as the 1975 World Health Assembly (WHA) adopted global programme 'Health for all by the year 2000' [2] and the 1988 institution of the Global Blood Safety Initiative (GBSI) [3], a joint effort of the World Health Organisation (WHO), the International Red Cross (IRC), the International Society of Blood Transfusion (ISBT) and the World Federation of Hemophilia (WFH). The latter initiative specifically focused on the safety and sustainability of the blood supply on national level through a global approach, triggered by the devastating AIDS epidemic.
