Radiation Safety Incidents are becoming more infrequent due to improvements in regulations, equipment, procedures and training of radiation workers. None the less, the potential for incidents still exists. While the infrequency of incidents is a major advantage, there are fewer opportunities for experience. It is therefore more important to share the details of incidents that occur. With information in hand, RSO personnel can better prepare with experiences of others and when opportunity permits by simulating actual incidents. This paper is limited to the details of the handling of three specific incidents and does not address other necessary tasks such as regulatory requirements, dosimetry monitoring, personnel qualifications and assignment, training and details of equipment involved. These are topics of separate papers and more important specialized training in source retrieval. It is however, note worthy that in accordance with Company policy, retrievals are not the task of a single individual, but that of a team. Though one individual has the final decision and responsibility, the team evaluates, analyzes, determines retrieval techniques and executes the retrieval. In this way, the benefits of experience are best utilized and disseminated and the principals of ALARA are maintained.
