METHOD FOR COMBINED LASER PHOTODESTRUCTION OF PYOGENIC GRANULOMA OF THE SKIN IN CHILDREN

FIELD: medicine.;SUBSTANCE: invention relates to medicine, namely, to surgery, and can be used to treat pyogenic granuloma (PG) of the skin in children. For this purpose, interstitial selective laser photodestruction of PG is performed with monochrome laser radiation with a wavelength of 0.97 mcm in two consecutive stages. At the first stage, the entire exophytic part of PG protruding above the surrounding skin is subjected to laser photodestruction by remote technique with a distance from the end of the manipulator to the surface of PG of 0.75±0.25 cm, in scanning mode at a rate of 1.0±0.25 cm/s. If the PG blood flow rate is from 15 to 25 cm/s, a power density of 15 to 25 J/cm2 is therein used, and if the PG blood flow rate is from 26 to 35 cm/s, a power density of 26 to 35 J/cm2 is used. At the second stage, the basal vessels of PG are subjected to laser photodestruction. For this purpose, a puncture manipulator is inserted through the coagulated PG tissue, simultaneously generating laser radiation to a depth of 1.0 mm±0.5 mm, and performing photodestruction in the direction of localisation of the basal vessels of PG by a linear technique. If the blood flow rate is from 15 to 25 cm/s, a power of 4.0 W is therein used, and if the blood flow rate is from 26 to 35 cm/s, a power of 5.0 to 6.0 W is used.;EFFECT: method ensures consistent, controlled, radical photodestruction of all pathological structures of PG, namely the exophytic part and the basal vessels involved in the haemodynamics thereof, while maintaining the viability of the surrounding tissues.;1 cl, 3 ex, 15 dwg
