A submucosal-mucosal flap is separated by 1/3 of the circumference of the anus 1 cm above the internal fistular opening transsecting the fistular passage, followed by treating the internal fistular opening using an electrocoagulator and a Volkmann curette. The defect of the internal fistular opening is sutured with a Z-shaped suture from the side of the submucosal layer.
