The patients health record card is at least partly made of embossable plastics material. The card has an aperture recess etc to allow insertion and locking in place of a microfilm. The inserted microfilm records the history of the illness, medication, innoculations, personal or medically important data, professional and military qualifications etc. It can be secured by welding or gluing, or the card can comprise two separable halves which can be secured together, holding the microfilm between them.
