Pilocarpine derivatives of formula I, IMAGE I wherein R1 to R5 are as defined in the description, are useful in the treatment of senile dementia, Alzheimer's disease, confusional conditions in the elderly, myasthenia gravis, schizophrenia, mania or glaucoma.
