PROBLEM TO BE SOLVED: To provide accurate information required for an emergent medical treatment, and make a patient smoothly receive a diagnosis and medical treatments at a hospital abroad, by listing information, which can be notified to his home doctor by translating into Japanese within 24 hours, in English, in addition to the medical treatment information of the patient listed by the home doctor. ;SOLUTION: On the surface of a paper 2, a comment 10 including a name 3, a disease name 6, a normally taken medicine (medicine for the medical treatment) 1, and a clinical recording 9 as general information, and initial examination data, latest examination data, information in details for asthma, and information in details for gastric ulcer or the like are listed in English based on the medical treatment information of a patient, which is listed by his home doctor. Also, separately from the general information, in a lower column of the paper main body 2, information 12 which can be notified to the home doctor by translating into Japanese within 24 hours, is listed in English. By such a diagnosis written in English for travel, when being suddenly hit by a disease on the site during a travel, even if the patient cannot speak English, a doctor of a hospital, to which the patient is carried, can quickly and correctly grasp a chronic disease or the like of the patient.
