PPROBLEM TO BE SOLVED: To create a document distribution confirmation sheet with multi-destination status and/or multi-service status information. PSOLUTION: The confirmation sheet which is created according to transmission of a document to at least one destination using at least one service, includes a status part which shows at least one of services to be used for transmission of at least one service and of at least one destination, destination for the transmission, transmission status, and at least one of information elements regarding to the transmission, and the document is transmitted as a single job using multi-services further/or to multi-destinations. Thumbnail images of one or more pages for multi-destinations and/or multi-service jobs are included in the confirmation sheet. If the element of the multi-destinations or the multi-service jobs are not completed at the time of request of the confirmation sheet, the confirmation sheet can include various structures and information for obtaining the job status later.
