A method of arrangement of data on the carrier of medical record documentation includes production of the form of medical record documentation, filling of blanks of form with necessary information, provision of basic form with one additional self-copying form, provision of basic and additional forms with a protrusion in the form of semicircle, provision of upper and/or lower part of the form with adhesive tape.
