PROBLEM TO BE SOLVED: To facilitate registration of a form easy to reuse, concerning a medical examination information input program, device and method.SOLUTION: The medical examination information input device is configured to display a sentence example to a user and receive a correction operation to the displayed sentence example from the user. The medical examination information input device is configured to compare an after-reflection sentence example to which the received correction operation is reflected with a pre-reflection sentence example before the correction operation is reflected to thereby identify a different point of the after-reflection sentence example differed from the pre-reflection sentence example. The medical examination information input device is configured to insert a predetermined symbol to the identified different point and register the after-reflection sentence example to which the predetermined symbol is inserted to a predetermined memory as a form.
