An apparatus (10) for using biomarkers for predicting one or more TNF-inhibitor responses in patients includes a measuring arrangement (such as a sequencer) (30) for processing one or more biological samples (20) to generate corresponding measurement data (40), a data processing arrangement (50) operable to obtain biological data derived from a plurality of samples from one or more databases (60), wherein the data processing arrangement (50) applies a leave out-one validation algorithm to the data from the plurality of samples to obtain probability indications for each sample related to response to the one or more biomarkers, wherein the data processing arrangement (50) is arranged to identify a subset of samples comprising biomarkers corresponding to a pre-determined regression threshold criterion (P) to obtain a preferred set of biomarkers, and wherein the data processing arrangement (50) is arranged to restrict the search space to known TNF-inhibitor predictors. The patients may be suffering from rheumatoid arthritis, inflammatory bowel disease, systemic lupus erythematosus or psoriasis, and the preferred set of biomarkers may comprise the protein ratio of sICAM1/CXCL13, the genotypes at loci rs12570744, rs2814707 and rs3849942 and the expression levels of CX3CR1 and CYP4F12.
