PROBLEM TO BE SOLVED: To improve the accuracy of input information in an inquiry sheet.;SOLUTION: An inquiry information input support program receives an input of patient's identification information, receives a selection of a medical examination department which is to be consulted by the patient, and identifies attribute information of the patient included in medical chart information of the patient with reference to a storage unit, which stores the identification information of the patient and the medical chart information of the patient in association with each other. Also, the inquiry information input support program identifies past inquiry input information the patient, if any, with reference to the storage unit. Then, during display of a screen for receiving an input of an inquiry of the selected medical examination department, the inquiry information input support program displays the screen with items corresponding to the attribute information and the past inquiry input information in an inputted state.;SELECTED DRAWING: Figure 14;COPYRIGHT: (C)2019,JPO&INPIT
