A system for enabling the delivery of healthcare services is provided. Thesystem has a server with a database thatstores an electronic community care record (ECCR). The system also has adirecting workstation. The directing workstation has a userinterface for allowing a licensed healthcare professional to access the ECCR.The system further has a mobile device. The mobile devicehas a user interface for allowing a healthcare assistant to access the ECCR.The ECCR has an entity history index that contains datacorresponding to an action performed on the user interface of the directingworkstation and the mobile device.
