A 37 year old man died after falling 15 feet when the vertical lift aerialplatform he was operating tipped over. The victim worked for a heating and air conditioning company which employed 33 workers. The company was installing air conditioning units and related electrical wiring at a newly constructed department store in Virginia. The victim was going to string electrical wiring above the grid work for suspended ceiling panels. He gained access to the ceiling by hydraulically operated vertical lift aerial platform. The victim mounted the machine and raised it to the 15 foot high position. He then started traveling in reverse to the work area. As he traveled, one of the four wheels dropped into a 14 inch deep unguarded hole in the floor, causing the machine to tip over. The victim was thrown from the equipment, striking his head on the concrete floor. He died of blunt force trauma to the head. It was recommended that all floor holes be guarded and/or covered, that safety requirements for operating self propelled elevating work platforms be followed, that written safety programs be reviewed with workers, and that rental company personnel explain safety procedures and provide training on the proper use of vertical lift aerial platforms.
