The manual provides instructions for the preparation of data entry sheets for the licensee event report (LER) file. It is a revision to an interim manual published in October 1974in 00E-SS-001. The LER file is a computer-based data bank of information using the data entry sheets as input. These data entry sheets contain pertinent information in regard to those occurrences required to be reported to the NRC. The computer-based data bank provides a centralized source of data that may be used for qualitative assessment of the nature and extent of off-normal events in the nuclear industry and as an index of source information to which users may refer for more detail.
