Valutazione dell'Appropriatezza d'Uso dell'Ospedale: Il Caso dei Presidi Ospedalieri di Lipari e Pantelleria. (Evaluation of Appropriateness of Hospital Use: The Case of Lipari and Pantelleria Hospitals).

The aim of the study is to assess the appropriateness of hospital use in Lipari and Pantelleria hospitals by means of PRUO (Protocol for Hospital Use Revisions) and of an adapted version to the Sicilian context. A retrospective study has been carried out on a sample of medical records of patients admitted to adult and paediatric wards during 2008-2010 years. In Lipari Hospital, 16% of ordinary admissions and 23,8% of stays are inappropriate; in Pantelleria Hospital, the percentage of inappropriate admissions is 8,5, while for stays it is almost the double (16,1%). Admission seems to be a predictor of index day. Lack in organisational structure, in particular the wait for performing a diagnostic test or consultation while in the hospital, are found to be the main reasons for inappropriate admissions and stays. The inappropriate hospital use is mainly related to day hospital, with 47% of inappropriate admissions in Lipari and 75% in Pantelleria.
