Fatal Accident Circumstances and Epidemiology (FACE) Report: Nineteen Year Old Electrocuted by a 440 Volt Trolley Wire

On Dec. 8, 1984, at approximately 3 p.m., a 19 year-old employee of a mine service equipment company was attempting to get a part from a metal pallet storage rack. The employee climbed the metal rack (approximately 20') and came into contact with a 440-volt trolley wire that supplies power to an overhead crane.
