P5. Application of trigger tool methodology to the detection of preventable ADEs associated with warfarin therapy-a pilot study

The study has provided us with preliminary data on the usefulness of the trigger tool as a means of delecting warfarin-related ADEs within the Trust. Using INR > 4 as a trigger identified more patients with actual ADEs than the trigger of INR > 6 used by other investigators. Further refinements to the methodology and data collection are required. The need to increase awareness of safe anticoagulant prescribing, particularly with respect to concurrent use of interacting drugs, has been highlighted.
