Authors' Rejoinder to Commentaries on 'Measurement in clinical trials: A neglected issue for statisticians?'

We hardly dared hope when we submitted our paper to Statistics in Medicine that it would attract such expert attention so quickly. We are extremely grateful to the editors for having arranged this and to those who have commented on it. We accept that our paper was somewhat combative: our intention was to generate debate. We thus can have no complaint if others have taken us to task. We cannot reply to all of the points raised but there are some individual comments that we would like to pick up.
