Los Angeles - The University of California at Los Angeles has issued new laboratory safety initiatives prompted by a flash fire incident that killed a university research assistant in January. On Dec. 29, 2008, UCLA staff research assistant Sheharbano "Sheri" Sangji was attempting to transfer t-butyl lithium from one container to another when the syringe broke, exposing the chemical to air and causing it to ignite. Sangji died of burns she received during the fire 18 days later. Cal/OSHA fined UCLA $31,875 in May, stating Sangji was not adequately trained and was not using proper personal protective equipment.
