In the aftermath of a March 2005 explosion that killed 15 workers and injured 180 at a BP Products North America refinery in Texas City,TX,an independent panel investigating the incident was tasked with determining BP's level of corporate safety oversight of U.S.refineries the company had obtained through mergers and acquisitions.Four of BP's five North American refineries,including Texas City,were acquired through mergers with the former Amoco and ARCO corporations.The BP U.S.Refineries Independent Safety Review Panel,led by former Secretary of State James Baker III,found material process safety deficiencies at all of BP's U.S.refineries.
