I-gel supraglottic airway for rescue airway management and as a conduit for tracheal intubation in a patient with acute respiratory failure.

A 54-year-old male presented as an emergency admission to Intensive Care Unit with a pneumonia. He had significant comorbidities including COPD, right heart failure, type 2 diabetes, increased BMI of 44, and obstructive sleep apnoea treated with non-invasive positive pressure ventilation at home. Shortly after admission he developed acute respiratory distress and became hypoxic, hypercarbic and confused. He was predicted to be a difficult intubation but his poor compliance limited our options for the intubation.
