In recent decades, the evolution of medical knowledge in several specialties has been amazing. The progress in the understanding of pathophysiological backgrounds has led to great therapeutical advances in intensive care, oncology and transplantation medicine. Also, our insight into the course of pulmonary disease, from asthma and COPD to the interstitial pneumonias, has rapidly increased and urged a paradigm shift in therapeutical approaches [1]
