ICD-10: Initial vs Subsequent Encounter

The codes for complications of care also require 7th characters to indicate the encounter. In the case of a complication, the 7th character selection is based on whether the patient is receiving active treatment for the complication, not for the underlying condition. For example, an internal-external biliary drainage catheter is placed due to biliary obstruction. If the catheter subsequently becomes infected, the code for the infection will be reported with a 7th character for "initial encounter" as long as the patient is receiving active treatment for the infection (not the biliary obstruction).
