Every system designed and administered by humans is subject to error. ARRT's systems are no different, and errors occasionally occur despite our best efforts to avoid them. Although the ARRT prides itself in designing processes that avoid mistakes, we take equal pride in how we handle mistakes when they occur. This column outlines ARRT's approach to problem-solving using a specific example.
