A confined space fatality on a fuel filling station construction project

A contractor's supervisor entered an underground storage tank at a motor fuel retail construction site to carry out tank cleaning. He was not using proper protective equipment. The tank had contained motor fuel and had not been internally gas tested. He was immediately overcome by fumes and collapsed. When emergency personnel retrieved him from the tank about one hour later, he was dead.
